Registration
Please come in and pick up forms to fill out. Please check the list below for items you must bring. If you have any questions, please feel free to call the office, (408) 377-2532.
When you return the forms you will need to bring the following items with you:
- PROOF OF DATE OF BIRTH - (Birth Certificate)
- IMMUNIZATION ACCORDING TO CALIFORNIA STATE LAW.
- TWO PROOFS OF RESIDENCY (PG&E bill, lease agreement or property tax bill, etc.)
